| Join the Prevention Think Tank National Registry of Prevention Professionals
(NRPP) If you
would like to be a member of the National Registry of Prevention Professionals (NRPP)you can join for $35 per year. Complete the sections below to be included in the National
Registry of Prevention Professionals. Here are the four (4) steps:
1) Send a check for
$35 payable to the Prevention Think Tank, Inc. and write "registry" in the memo
field. Use the address that is listed in the menu item Contact
Us. OR -- use this link [PayPal Link]
to pay by credit card (or an electronic check) with PayPal. Sorry, we cannot accept
purchase orders or do invoicing.
2) You will also need to
sign and send in the Code of Ethical Conduct.
You can download the form from this website. Send it in with your check, or if paying by
credit card you can fax it to 866-481-0735.
3) Fill out the electronic
form below and press SEND. We need this information electronically to avoid data-entry
errors on our end and to speed up the process.
4) Optional - If you would
like one or two (1 or 2 only) graphics included in your listing (such as a logo or a
picture), please email the graphic "jpeg" or "gif" file to pttinc@preventionthinktank.org and specify in the body of the
message the NRPP entry for which the graphic is intended (full name and email). You can
also mail hard copy pictures and/or logos to be scanned, but if you want them returned to
you be sure to include a stamped addressed return envelope. Use the address that is listed
in the menu item Contact Us.
All information requested
below is OPTIONAL except your name, and you need to provide at
least one method of contact (address, telephone, fax and/or email). Hint: For your privacy, we suggest
that you do not use your home address unless you also use it as your business address.
